Human authority handoff

Pending HUMAN authority requests live in data/human-actions.yaml.

These are proposals, not factual truth. Accepting them records HUMAN authority through the same services as direct CLI/TUI.

Stale proposals (target already HUMAN-final / DONE) become SUPERSEDED.
